Job Description

We are looking for experienced Licensed Practical Nurses (LPNs) to join our dynamic team at ChristianaCare HomeHealth. As a key member of our team, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement goal-oriented, patient-centered care plans.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Must possess a current DE LPN license, or compact state license.
