We are looking for experienced Home Care LPNs to join our team at ChristianaCare. Responsibilities

• Provide skilled nursing care and treatments to geriatric patients in the community.
• Implement patient-centered care plans in line with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
