Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As part of our HomeHealth division, you will be responsible for providing skilled nursing care to geriatric patients in the community. Our organization offers a state-of-the-art Electronic Health Record system and comprehensive training to ensure your success in the role.
Responsibilities
- Provide nursing care and treatments to patients in accordance with care plans.
- Contribute to goal-oriented care plans and assist in discharge planning.
Requirements
- High School Diploma or GED required.
- Current DE LPN license, or compact state license, is mandatory.
