Job Description
We are currently seeking experienced Home Care LPNs to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in collaboration with the healthcare team.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
