Job Description
We are looking for an experienced Home Care Licensed Practical Nurse (LPN) to join our team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. Your role will involve implementing patient-centered care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
