Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and contributing to discharge planning.
Responsibilities
- Deliver skilled nursing care to geriatric patients.
- Implement patient-centered care plans.
- Participate in discharge planning.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
