Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans and discharge plans.
- Support patients in New Castle County, DE on day or night shifts, with weekend and holiday rotations.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
Benefits
- $22.00 - $28.00 per hour pay range.
