Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients, implement care plans, and participate in discharge planning under supportive leadership.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
