Job Description
Are you an experienced LPN looking to join a dynamic team in Home Health Care? ChristianaCare is currently seeking Full Time, Part Time, and Per Diem LPNs to support patients in New Castle County, DE. As part of our team, you will provide skilled nursing care to geriatric patients in a community setting, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
