Registered Nurse (RN) - Home Health - $32-58 per hour

Atrium Health is looking for a dedicated Registered Nurse (RN) for Home Health services in Charlotte, North Carolina. This role offers the opportunity to provide world-class healthcare to adult and geriatric patients in the comfort of their homes. As a member of the Atrium Health At Home team, you will play a crucial role in ensuring compassionate care and advanced services that meet the individual needs of each patient.

Responsibilities

• Manage an assigned group of patients, overseeing post-acute services in the home setting.
• Provide direct patient care, supervise LPNs and HHAs, and coordinate care with a multidisciplinary team.
• Develop and implement effective care plans to promote patient health and safety outcomes.
• Evaluate patient progress towards set goals and ensure high-quality documentation.

Requirements

• Graduate from an accredited School of Nursing, with a preference for a BSN.
• Current RN license and BLS certification required.
• Additional training or certifications as needed within the department.
• Valid driver's license necessary for patient visits.
