Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

Atrium Health is looking for a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Charlotte, North Carolina. As a Home Health Nurse, you will provide world-class healthcare to adult and geriatric patients in the comfort of their own homes.

Responsibilities

• Manage an assigned group of patients, overseeing post-acute services in the home setting.
• Provide direct patient care and supervision of LPNs and HHAs according to established care plans.
• Coordinate care with a multi-disciplinary team and act as the patient's liaison with healthcare providers.
• Ensure timely and quality patient care and documentation to meet agency goals.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license and BLS certification required.
• Ability to prioritize patient needs, ensure patient safety, and promote health outcomes.
• Valid driver's license required.
