Job Description

We are looking for a dedicated Social Worker to join the team at LifeBridge Health in Baltimore, MD, specifically at Levindale Hospital. As a full-time Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, addressing various common challenges they may face. Our ideal candidate will have a Bachelor's in Social Work (BSW), with a preference for a Master's degree and LCSW-C certification.

Responsibilities

• Offer social work services and psychosocial assessments to patients, residents, and families.
• Assist with adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on medical diagnoses, aging, discharge planning, and end-of-life issues.

Requirements

• Bachelor's in Social Work (BSW); Master's degree is preferred.
• LCSW-C certification is preferred.
• LGSW certification.
