Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients and families in coping with medical diagnoses and treatment plans.
- Develop discharge plans and provide support during the transition process.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
