Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common challenges such as adjustment to institutional living and discharge planning.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address issues related to adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling on topics such as understanding medical conditions, aging, and end-of-life care.
- Collaborate with healthcare teams to create effective discharge plans.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred.
- LGSW certification required.
- Prior experience in clinical social work.
