Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients, residents, and their families. The role involves addressing common challenges such as adjusting to institutional living, financial concerns, family dynamics, medical procedures, aging-related issues, discharge planning, and end-of-life support.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Offer counseling on various issues such as family relationships, financial concerns, and end-of-life support.
- Collaborate with healthcare teams to support patient care and well-being.
- Ensure patients and families have access to resources for ongoing support.
Requirements
- Bachelor's in Social Work (BSW). Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Prior experience in a healthcare setting is advantageous.
