LifeBridge Health is seeking a full-time Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling services.
• Assist patients and families with discharge planning and coping with medical diagnoses.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Advocate for patients' rights and access to resources.

Requirements

• Bachelor's in Social Work (BSW), Master's degree preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
• Excellent communication and interpersonal skills.
