• Conduct home visit Physical Therapy assessments.

• Document patient records efficiently using EMR systems.
• Collaborate with patients to develop rehabilitation plans.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Proficient in EMR documentation.
• CPR Certification.
