Responsibilities

• Conduct home visits to perform Physical Therapy assessments on referred patients.
• Provide direct patient care to evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Document patient information accurately using EMR systems.
• Maintain CPR certification to ensure patient safety.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid and unencumbered Physical Therapy license for New York.
• Experience in conducting assessments and providing patient care.
• Familiarity with EMR documentation (experience with Hello Note is a plus).
