Physical Therapist Responsibilities:

• Perform home visit physical therapy assessments.
• Deliver direct patient care to assess medical conditions and functional capabilities.
• Document patient records accurately using EMR systems.

Physical Therapist Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Proficient in EMR documentation.
• CPR Certification.
