Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and potential for rehabilitation.
• Provide direct patient care and functional capability assessments.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• Ensure compliance with CPR certification and licensure requirements.

Requirements

• Graduated from an accredited PT college or university.
• Unencumbered PT license for New York.
• Experience with home visit assessments is preferred.
• Strong communication and interpersonal skills.
