Physical Therapy Specialist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments, provide direct patient care, and assess medical conditions for rehabilitation potential.
• Proficient in EMR documentation and CPR Certification.
