We are looking for a dedicated Physical Therapy Specialist to join our team at Paradigm Rehab. As a Physical Therapist, you will play a crucial role in helping individuals improve their quality of life by conducting home visit assessments and providing direct patient care.

Responsibilities

• Conduct Physical Therapy assessments and provide direct patient care.
• Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation.

Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• CPR Certification.
