• Conduct home visit Physical Therapy assessments.
• Assess medical conditions, functional capabilities, and limitations.
• Provide direct patient care to support rehabilitation.
• Proficient in EMR documentation.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Unencumbered PT license for New York.
• Proficient in EMR documentation.
• CPR Certification.
