Rehabilitation Therapist Responsibilities:

• Conduct home visit physical therapy assessments.
• Provide direct patient care and assess medical conditions.
• Document patient records using EMR systems.

Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
