Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions, functional capabilities, and potential for rehabilitation.
• Provide direct patient care to assist in the rehabilitation process.
• Document patient progress accurately using EMR systems.

Requirements

• Graduate of an accredited Physical Therapy program.
• Valid Physical Therapy license in New York.
• Proficient in EMR documentation.
• CPR Certification.
