Paradigm Rehab is looking for a dedicated Rehabilitation Therapist to join our compassionate team. As a Rehabilitation Therapist, you will conduct home visits, perform assessments, and provide direct patient care to support rehabilitation and improve functional capabilities.

Responsibilities

• Perform Physical Therapy assessments and deliver direct patient care.
• Document patient progress using EMR systems.
• Maintain a compassionate and supportive approach to patient care.
• Collaborate with the healthcare team to optimize patient outcomes.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Familiarity with EMR documentation.
• Current CPR Certification.
