Responsibilities
- Conduct home visit assessments and provide direct patient care.
- Assess medical conditions, functional capabilities, and potential for rehabilitation.
- Document patient information using EMR systems.
Requirements
- Graduate from an accredited Physical Therapy program.
- Hold an unencumbered Physical Therapy license in New York.
- Proficiency in EMR documentation.
- CPR Certification.
