Licensed Practical Nurse MDS Coordinator

Job Description

We are looking for a Licensed Practical Nurse MDS Coordinator to join Genesis Health, a leading provider of healthcare services. As an LPN MDS Coordinator, you will participate in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement. Your role will involve directing the interdisciplinary RAI process, developing resident Care Plans, and training team members on clinical content and electronic MDS completion.

Requirements

• LPN license in good standing.
• Previous experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and regulatory guidelines.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
• Proficiency in RAI/PPS processes.
• Current BLS/CPR certificate.
