Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a licensed Practical Nurse to join our team as a Minimal Data Set Coordinator. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for completion, encoding, and state transmission of MDS.
• Contribute to the development of personalized resident Care Plans, addressing resident needs and expected outcomes in accordance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or related healthcare settings.
• Proficiency in general, rehabilitative, and restorative nursing practices, as well as relevant laws and regulations.
• Computer skills including proficiency in Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferable.
• Current BLS/CPR certification required.
