Minimal Data Set Coordinator - LPN

Job Description

We are looking for a skilled Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN) to join Genesis HealthCare. As a valued member of our team, you will play a vital role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, adhering to completion guidelines, encoding, and electronic state transmission of the MDS.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and expected outcomes in line with Federal and State regulations.
• Provide training to new team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in computer skills, including windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
