Job Description
Genesis Healthcare is currently seeking a Nurse-LPN MDS Coordinator to join our team. As a leading provider of healthcare services, we are dedicated to delivering high-quality care and compassion to improve the lives of our patients and residents. In this role, you will play a vital part in the Resident Assessment Process, ensuring appropriate patient care and nursing center reimbursement through the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs).
Position Highlights
- Direct the RAI process and ensure accurate completion and transmission of MDS.
- Collaborate in developing individualized resident Care Plans in compliance with Federal and State regulations.
- Provide training on clinical content and electronic completion of MDS items to interdisciplinary team members.

Requirements
- LPN license and previous healthcare facility experience.
- Knowledge of nursing practices and MDS completion.
- Computer skills, proficiency in RAI/PPS processes, and BLS/CPR certification.
