As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing MDS and CAAs to ensure quality patient care and proper reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans following Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license with previous healthcare facility experience.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Knowledge of laws, regulations, and guidelines in long-term care.
• Computer skills, including Windows applications; AccuMed experience preferred.
• Prior experience as an MDS Coordinator with proficiency in RAI/PPS processes.
• Current BLS/CPR certificate.
